ABSTRACT

Fundamentos: Em várias cardiopatias a anticoagulação previne eventos tromboembólicos. Varfarina é o anticoagulante oral mais utilizado, entretanto sua eficácia e segurança dependem do monitoramento do tempo de protrombina (TP) e sua padronização pelo INR. Manter INR adequado é um desafio, especialmente em população de baixos níveis educacional e socioeconômico. Ambulatório especializado em anticoagulação representa importante aliado.Objetivo: Avaliar o controle do INR em pacientes acompanhados em ambulatório de anticoagulação.Métodos: Durante dois meses foram selecionados pacientes que utilizavam varfarina e eram acompanhados em ambulatório especializado no controle de TP. Os pacientes são assistidos em intervalo máximo de quatro semanas, recebem orientação sobre riscos e benefícios da anticoagulação e têm carteira específica para o acompanhamento. Foi considerado INR adequado valores entre 2,0 e 3,5. Utilizado o teste t de Student e considerado significativo p<0,05. Resultados: Avaliados 96 pacientes, sendo 50 (56,25 %) homens; média de idade 59±14,9 anos. A indicação para anticoagulação foi fibrilação/flutter atrial em 55,21 %; prótese valvar em 26,04 %; tromboembolismo prévio em 9,38 % e trombo intracardíaco em 9,37 %. O tempo de acompanhamento médio foi 361,26±216,34 dias, e 66 (68,75 %) pacientes apresentaram INR adequado. A dose média diária de varfarina foi 4,93±3,13 mg/dia. A média de idade, o tempo de acompanhamento e a dose média diária foram semelhantes nos dois grupos: com INR adequado e com INR inadequado. Conclusão: A anticoagulação orientada por equipe especializada é efetiva e segura em pacientes com baixa renda e baixo nível educacional, permitindo que seja prescrita para esses pacientes.


Background: In several cardiopathies anticoagulation prevents thromboembolic events. Although warfarin is the most widely used oral anticoagulant, its efficacy and safety depend on monitoring the prothrombin time (PT) and its standardization by INR. Maintaining adequate INR is challenging, especially in populations with low educational, social and economic levels. Clinics specializing in anticoagulation are important allies.Objective: To evaluate INR control among patients monitored at an anticoagulation clinic. Methods: For two months, patients taking warfarin were selected and monitored at a PT control clinic, seen at least every four weeks and receiving guidance on the risks and benefits of anticoagulation, with specific monitoring cards. Appropriate INR values were taken as between 2.0 and 3.5, using the Student t test and a significance of p<0.05.Results: Having evaluated 96 patients, with 50 men (56.25 %) and a mean age of 59±14.9 years, the indication for anticoagulation was atrial fibrillation / atrial flutter for 55.21 %; prosthetic valve for 26.04 %; previous thromboembolism for 9.38 % and intracardiac thrombus for 9.37 %. The median follow-up time at the clinic was 361.26±216.34 days, and 66 (68.75 %) of the patients had an appropriate INR. The mean daily dose of warfarin was 4.93±3.13 mg/day. The average age, monitoring time and mean daily dose were similar for both groups, with adequate and inadequate INR. Conclusion: Anticoagulation guided by specialized staff is effective and safe for patients with lower income and educational levels, allowing it to be prescribed for these patients.

ABSTRACT

Paciente com sinais e sintomas de insuficiência cardíaca direita de etiologia desconhecida, transferida para tratamento em hospital de referência da zona leste de São Paulo, com diagnóstico de pericardite constritiva calcificada, tratada cirurgicamente. Essa patologia caracteriza-se por processo de calcificação irreversível do pericárdio, e o tratamento cirúrgico é a alternativa para o controle dos sintomas e a melhora da qualidade de vida dos pacientes. Esse caso chamou a atenção pela extensão da calcificação e por seu local de distribuição, atingindo o septo interventricular, fato que dificultou o diagnóstico pelo aspecto inusitado das imagens, deixando dúvida se haveria outra doença associada.


A patient with signs and symptoms of right heart failure of unknown etiology was referred to a referral hospital in the eastern area of the city of São Paulo with a diagnosis of calcified constrictive pericarditis and was treated by surgery. This pathology is characterized by an irreversible process of pericardium calcification, and surgery is the only alternative to control the symptoms and improve patients' quality of life. This case drew special attention due to the extensive calcification involving the interventricular septum. The unusual aspect of the images has made the diagnosis difficult and raised doubts about the existence of an associated disease.


Pacientes con signos y síntomas de insuficiencia cardíaca derecha, de etiología desconocida, trasladados para recibir tratamiento en un hospital de referencia en la región este de São Paulo, con diagnóstico de pericarditis constrictiva calcificada, tratada quirúrgicamente. Esta patología se caracteriza por un proceso de calcificación irreversible del pericardio, y el tratamiento quirúrgico es la alternativa para el control de los síntomas y la mejora de la calidad de vida de los pacientes. Este caso llamó la atención por la extensión de la calcificación y por su local de distribución, alcanzando el tabique interventricular, lo que dificultó el diagnóstico por el aspecto inusual de las imágenes, dejando dudas sobre si no habría otra enfermedad asociada.

ABSTRACT

OBJECTIVE: To identify predictors of low cardiac output and mortality in decompensated heart failure. INTRODUCTION: Introduction: Patients with decompensated heart failure have a high mortality rate, especially those patients with low cardiac output. However, this clinical presentation is uncommon, and its management is controversial. METHODS: We studied a cohort of 452 patients hospitalized with decompensated heart failure with an ejection fraction of <0.45. Patients underwent clinical-hemodynamic assessment and Chagas disease immunoenzymatic assay. Low cardiac output was defined according to L and C clinical-hemodynamic profiles. Multivariate analyses assessed clinical outcomes. P<0.05 was considered significant. RESULTS: The mean age was 60.1 years; 245 (54.2 percent) patients were >60 years, and 64.6 percent were men. Low cardiac output was present in 281 (63 percent) patients on admission. Chagas disease was the cause of heart failure in 92 (20.4 percent) patients who had higher B type natriuretic peptide levels (1,978.38 vs. 1,697.64 pg/mL; P = 0.015). Predictors of low cardiac output were Chagas disease (RR: 3.655, P<0.001), lower ejection fraction (RR: 2.414, P<0.001), hyponatremia (RR: 1.618, P = 0.036), and renal dysfunction (RR: 1.916, P = 0.007). Elderly patients were inversely associated with low cardiac output (RR: 0.436, P = 0.001). Predictors of mortality were Chagas disease (RR: 2.286, P<0.001), ischemic etiology (RR: 1.449, P = 0.035), and low cardiac output (RR: 1.419, P = 0.047). CONCLUSIONS: In severe decompensated heart failure, predictors of low cardiac output are Chagas disease, lower ejection fraction, hyponatremia, and renal dysfunction. Additionally, Chagas disease patients have higher B type natriuretic peptide levels and a worse prognosis independent of lower ejection fraction.

A insuficiência cardíaca é uma síndrome complexa, principalmente quando associada a múltiplas comorbidades, como é comum no idoso. Compreender as interações e decidir o tratamento, considerando as peculiaridades dos idosos, promovem melhora da sobrevida e da qualidade de vida.
